Letter from the Executive Director

Dear Eagle Lake Resident and Resident Families,

My name is Sam Onyebuchi, and I am the new Executive Director of Eagle Lake Village. I
wanted to reach out to you directly to ensure you heard this news of my appointment and to briefly
comment on service continuity, the future, and my background.

First, I am thankful for the opportunity given to me to serve at Eagle Lake Village, and I'm proud
of the legacies built here. I am equally enthusiastic in working with the able and
professional team already in place to continue to play an integral role in the daily life of the
Residents.

About my background, I started as a CNA 15 years ago at Claremont Manor in Claremont,
California. After a year, I was promoted to an Associate Executive Director in charge of Memory Care. Two years later, I became the Executive Director at Pilgrim's Place in Claremont. I was there
for six years, overseeing several expansions including the establishment of a memory care unit. My tour of duty took me to The Villas at San Bernardino where I became the Executive
Director, overseeing the Independent Living, Assisted Living, and Memory Care units. After four
years at the Villas, I was called to serve at ResCare California Inc. in January 2017, and I was there until August 2018. I briefly became the interim Executive Director at Park Vista
Senior Living in Thousand Oaks from August to December 2018.

I am a California certified RCFE Administrator. I have a Bachelors in Theology and Philosophy; a Masters in Clinical Psychology from La Sierra University, Riverside, CA; an Associate in
Law/Legal Studies from Boston University, MA. I am also currently pursuing my Ph.D. in
Healthcare Administration and Leadership from the University of San Diego, California. I am a
Fellow of the American Society on Aging. I am equally a Professional Member of the American College of Healthcare Executives as well as the National Association of Health Services Executives.

My greatest and undying passion is to serve the seniors and the elderly; to ensure they live a
quality and fulfilled life. Part of my philosophy is that I work at their homes, they own me, and I am their servant.

When I am not working, I read, travel, and write. Apart from serving at the assisted living communities, I am equally a pastor/member of the Seventh-day Adventist Church. I love God,
and I love people.

My contact info is attached, and I look forward to meeting you and sharing ideas to preserve and enhance Eagle Lake Village. I'll make time to meet with each of you and look forward to doing so.

Sincerely,

Sam Onyebuchi

Executive Director
